Directions for the Evaluation

To complete the evaluation, you will need a computer or mobile device with internet access, your username/email address, and password that you created upon registration for this conference to login. If you do not remember your password, please use the password reset function to obtain a new password.  


How to complete your Evaluation

  1. Click on the program link to access the evaluation page
  2. Login to the website using your username and password
  3. Read the evaluation materials, click on the appropriate ratings, and answer the questions
  4. Check the attestation box verifying completion of the Requirements for Successful Completion
  5. Click the SUBMIT button at the end of the evaluation
  6. You should be redirected to the the Certificates tab in your profile
  7. Under the Certificates tab in your profile, click the print but to obtain your Certificate of Successful Completion.
  8. Use your web browser to print your Certificate of Successful Completion or save to your computer files.
